runner on first, 1 out, groundball to short, throws to second, second baseman misses the throw, is the base runner required to slide

You are not required to slide. If the second baseman catches the ball and tries to turn a double play you have to "give" yourself up by either sliding or running out of the baseline or you can be called for interference, which in turn the batter is also out at first . Your not going to see this called very much though.
